POSITION SUMMARY:
This is a professional position providing case management services to clients involved with the Housing First Division. Targeted Prevention provides intensive case management services for families with dependent children who are not homeless and are housed, but are at-risk of becoming street homeless due to eviction or the loss of housing.
The Targeted Prevention Specialist will ensure that case management services focus on providing case management interventions that increase skills related to managing mental health and substance use issues as well as skills that increase housing stability. The Targeted Prevention Specialist will utilize natural supports, community supports, and input and resources from other community partners or professional agencies. The Targeted Prevention Specialist ensures services are delivered using standards of trauma informed care principles, and that plans and clinical interventions address issues regarding how trauma impacts family functioning, and that plans and interventions use trauma resolution skills. The Targeted Prevention Specialist will ensure that case management services occur in the community at a place convenient and comfortable for the youth and family. This is a field position. Responsible to meet productivity requirements of 1248 hours of direct face to face client contacts annually or a 104 hours per month.
