Job summary

El Paso Child Guidance Center is seeking a TCM Care Coordinator to deliver targeted case management and psychosocial rehabilitation services for clients in community settings. The role includes training in evidence-based methodologies and maintaining clinical documentation.

As part of EPCGC, you will work with a multidisciplinary team to develop treatment plans, coordinate care, and ensure timely assessments and follow-up for both children and adults in a supportive environment.

Qualifications

• Qualified Mental Health Professional (QMHP) certification or eligibility required.
• Experience in Targeted Case Management or related community-based services is preferred.
• Ability to document and maintain client records confidentially and accurately.

Responsibilities

• Provide Targeted Case Management services to eligible clients in community settings.
• Deliver family-oriented, individual and group psychosocial rehabilitation services as needed.
• Maintain accurate documentation and ensure timely updates in required databases.
