Job summary

Horizon Health Services in Lockport, NY is seeking a dedicated Targeted Case Manager to perform SDOH assessments, support rapid access to care, and connect patients with community services through outreach, telehealth, and in-person visits.

The role emphasizes working with behavioral health clients, building supportive relationships, and navigating EHR systems, with a Monday-Thursday 8am-5:15pm schedule and a grant-funded status.

Qualifications

• Bachelor's degree plus 6 months of relevant behavioral health experience required.
• Associates degree plus 1 year of relevant experience required.
• High School Diploma/equivalent plus 2 years of relevant experience required.
• Computer navigation skills and proficiency in Microsoft Office.

Responsibilities

• Complete Social Determinants of Health (SDOH) assessments for patients.
• Participate in Open Access process to assist patients with rapid access to care.
• Provide patient support through outreach calls, telehealth, and face-to-face visits to identify barriers and connect patients to community resources and services.
• Deliver transitional care with follow-up outreach to connect patients from inpatient, ER, or residential settings to appropriate providers.
• Connect individuals with necessary medical, social, legal, housing, and other supportive services.
• Assist patients in addressing gaps in care and achieving health goals.
• Retrieve and utilize data from EHR systems and health information networks.
• Build positive, supportive relationships with individuals with behavioral health disorders and families.

As a Targeted Case Manager at Horizon, you will….

• Build positive, supportive relationships with individuals who have behavioral health disorders and their families, respecting diverse ethnic, cultural, and socioeconomic backgrounds.
• Learn and adhere to Horizon Health Services policies and procedures, including compliance with privacy and confidentiality regulations.

Location:

This position is located at 637 Davison Road, Lockport NY.

Hours:

Monday through Thursday: 8am - 5:15pm, Friday: 8am - 1pm.

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $20.00 - $24.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*Please note: this is a grant-funded position.

Physical demands associated with this position include:

• Sitting for extended periods, alternating between standing and sitting
• Mobility required with repetitive wrist, hand, and finger movements
• Handling light duties, occasionally lifting objects up to 20 pounds
• Exposure to various environmental conditions including low/high temperatures, outdoor elements
