Job summary

Horizon Health Services in Lockport, NY seeks a patient-facing professional to support behavioral health and addiction services. You will perform SDOH assessments, aid rapid care access, and connect patients with needed resources through outreach, telehealth, and in-person visits.

You will work with EHR systems, engage with diverse communities, and follow Horizon policies to ensure compliant care delivery while contributing to a supportive, collaborative team environment.

Qualifications

• Bachelor's degree required or an Associate degree with 1 year of relevant experience; higher education may be substituted by experience.
• Experience in a behavioral health setting in a patient-facing role is required or implied by the degree path.
• Proficiency with Microsoft Office and computer navigation is expected.
• Familiarity with reporting and entry-level data analytics is preferred.

• Build positive, supportive relationships with individuals who have behavioral health disorders and their families, respecting diverse ethnic, cultural, and socioeconomic backgrounds.
• Learn and adhere to Horizon Health Services policies and procedures, including compliance with privacy and confidentiality regulations.

• Familiarity with reporting and entry level data analytics.

Location:

This position is located at 637 Davison Road, Lockport NY.

Hours:

Monday through Thursday: 8am - 5:15pm, Friday: 8am - 1pm.

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $20.00 - $24.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually. This position entails eligibility for a sign‑on bonus of up to $1,500 for your commitment to work at our Horizon Certified Behavioral Health Clinics (CCBHC) located at our Pine Avenue Recovery Center and Lockport Recovery Centers for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.

Physical demands associated with this position include:

• Sitting for extended periods, alternating between standing and sitting
• Mobility required with repetitive wrist, hand, and finger movements
• Handling light duties, occasionally lifting objects up to 20 pounds
• Local travel between worksites
• Exposure to various environmental conditions including low/high temperatures, outdoor elements
