ESSENTIAL JOB FUNCTIONS

  1. Conducts and documents comprehensive assessment of client and/or client's needs.
  2. Participates in development of client's service plan, consistent with assessment.
  3. Arranges for delivery of needed services, as identified in assessment.
  4. Assists client in accessing needed services through referrals or linkages.
  5. Monitors client's progress by making referrals, tracking appointments, performing follow-up, and reassessing needs.
  6. Performs advocacy activities on behalf of client, such as writing letters and encouraging self-advocacy.
  7. Prepares and maintains case records in accordance with standards.
  8. Provides case consultation with service providers.
  9. Communicates with supervisor regarding case management issues.
  10. Provides psychosocial education to clients and families.

EDUCATION

  • Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid or funding source.
  • Completion of state-required case management course within six months of employment.

EXPERIENCE

  • At least one year of full-time experience working directly with adults or ACT population in a human service setting, with relevant Master's degree, no experience required.

PHYSICAL DEMANDS/REQUIREMENTS

  • Lifting up to 20 pounds, frequent lifting/carrying of up to 10 pounds.
  • Exposure to threat of bodily harm and possibly blood or bodily fluids.
  • May involve 24-hour on-call responsibilities.
  • Reliable transportation required for travel.
