Job summary

Painted Grove Recovery seeks a dedicated Targeted Case Manager in Louisville to coordinate patient care across inpatient and outpatient services, with a focus on individualized, holistic treatment for substance use disorders.

The role requires a Bachelor’s Degree and TC M certification or training within 6 months, plus a valid driver’s license and driving record for a minimum of 3 years. The position is full-time and located at PSDF2 with standard business hours.

Qualifications

  • Bachelor’s Degree is required.
  • Targeted Case Management certification preferred or must complete training within 6 months of employment.
  • Valid, unrestricted driver’s license.
  • Clean driving record for at least 3 years.
  • Minimum 1 year experience working directly with adults or adolescents in a human service setting.
  • Commitment to a drug-free lifestyle and responsible alcohol use.
  • Personal passion or interest in substance abuse issues is strongly preferred.
  • Must meet all applicable State Regulations.

Responsibilities

  • Assess patient biopsychosocial stressors (housing, financial, legal, medical, employment) and develop strategies to address needs during and after treatment.
  • Facilitate case coordination, information sharing, aftercare planning, and court-related issues.
  • Maintain positive working relationships with support networks, government and community resources, employers, and referral sources.
  • Complete all necessary documentation related to FMLA and Short-Term Disability.
  • Maintain patient records by reviewing and completing case notes.
  • Prepare patient discharges by coordinating community services to maintain sobriety outside of a structured setting.
  • Participate in weekly multidisciplinary treatment team meetings to assess patient needs and discharge planning.
  • Engage in daily census management, coordinating discharge schedules and communication with treatment and administrative teams.
  • Continuously maintain professional licensure through Continuing Education.
  • Transport patients in company vehicle to and from Intensive Outpatient Programs (IOP).

Education

Bachelor's Degree

Overview

The Targeted Case Manager acts as a member of a multidisciplinary team to assess and coordinate patients’ care among multiple service providers during inpatient and outpatient treatment.

Essential Duties and Responsibilities
  • Assess patient biopsychosocial stressors (e.g., housing, financial, legal, medical, employment) and develop strategies to address these needs during and after treatment

Education, Experience, and Training
  • Bachelor’s Degree required
  • Previous Targeted Case Management experience or supervision preferred
  • Valid, unrestricted driver’s license
  • Clean driving record for at least 3 years
  • Minimum 1 year experience working directly with adults or adolescents in a human service setting
  • Commitment to a drug-free lifestyle and responsible alcohol use
  • Personal passion or interest in substance abuse issues strongly preferred
  • Must meet all applicable State Regulations
Certificates, Licenses, Registrations
  • Targeted Case Management (TCM) certification preferred, or must complete department-approved targeted case management training within 6 months of employment
