Job summary

Pathways Inc. - Kentucky is seeking a full-time Case Manager to support consumers with individualized treatment plans, coordinating behavioral health, medical, and social needs.

You will document accurately and maintain compliance across agency standards while collaborating with families, providers, and community partners. Required are a Bachelor’s degree in Human Services or related field and at least one year of post-graduate experience; prior behavioral health experience is preferred.

Qualifications

• Experience in a behavioral health setting preferred.

Responsibilities

• Provide case management services per individualized treatment plans
• Coordinate services to support behavioral health, medical, social, and community resources
• Maintain accurate, timely documentation compliant with laws and agency requirements
• Collaborate with consumers, families, providers, and community partners to ensure continuity of care
• Assist consumers in accessing community resources and benefits
• Participate in treatment team meetings and contribute to care planning
• Adhere to Pathways policies, procedures, and confidentiality standards
• Perform additional duties within the scope of the position

Salary:

$35,000 - $39,375 annually

Educational Requirements

Duties & Responsibilities

• Provide case management services to assigned consumers in accordance with individualized treatment and service plans
• Coordinate and monitor services to support consumers’ behavioral health, medical, social, and community resource needs
• Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements
• Collaborate with consumers, families, providers, and community partners to ensure continuity of care
• Assist consumers with accessing community resources, benefits, and supportive services
• Participate in treatment team meetings and contribute to care planning efforts
• Adhere to all Pathways policies, procedures, and confidentiality standards
• Perform additional duties as assigned within the scope of the position

Skills & Knowledge

• Experience working in a behavioral health setting preferred
• Strong communication, organization, and interpersonal skills
• Ability to manage multiple tasks and maintain accurate documentation
• Competency in using computers, electronic health records, and other electronic devices required for job duties
