Qualifications

  • Ability to analyze opportunities and problems, identify and evaluate alternatives, and develop effective approaches to address issues.
  • Ability to plan, implement, and evaluate goals and work plans.

Position Summary:

Ensure GWH associates receive services from WFD, strategic partners, and other community resources consistent with the mission, identified outcomes, and each associate's individualized plan outlining a path toward retention and growth.

Responsible for a variety of roles in order to provide individual associates, and when warranted, their families, with comprehensive, "single point of contact" support as part of a seamless service delivery design. Services provided to associates include life and work coaching, personal and career counseling, work assessments, mediation, coordination and advocacy with organizational and community resources, and case management to achieve self-sufficiency.

Essential Duties and Responsibilities:
  • Accountable for the associates' full Individual Development Plan (IDP) cycle, including assessing, planning, implementing, monitoring, and evaluating actions required to meet the associate's accommodation, retention, and career growth needs.
  • Effectively develop positive, empowering relationships with the associates through coaching, motivating, and educating.
  • Actively promote GWH programs to create awareness and demonstrate the potential they have in career and life advancement.
  • Fluent in the evidence-based career navigation model and the specific career pathways within GWH.
  • Fluent in local community resources and programs, and provide constant referrals to programs that foster the continued growth of the associate.
  • Maintain working knowledge of educational and training opportunities located inside and externally in the community.
  • Document coaching, with the associates through coaching notes in electronic case management systems.
  • Coordinate with associates and pre-schedule meetings.
  • Refer associates to outside sources or additional training services or social aids.
  • Establish a rapport with associates.
  • Intervene in crises.
  • Travel to multiple locations as needed.
  • Perform other duties as directed by management to support evolving organizational needs.

Minimum Qualifications:
  • A high school diploma or equivalent is required.
  • Ability to read, write, and communicate effectively in English to understand safety instructions, standard operating procedures, and workplacecommunications.
  • A minimum of one (1) year of experience in case management is preferred.
  • Extensive knowledge of community resources in the four sector areas: business, government, non-profit, and faith-based organizations.
  • Experience as a facilitator, instructor or career coach.
  • Good computer skills (proficiency in Microsoft Office).
  • Excellent written and verbal communication skills.
  • Ability to communicate and interact well with a variety of personality types.
  • Ability to analyze opportunities and problems, identify and evaluate alternatives, and develop effective approaches to address any issues.
  • Ability to plan, implement, and evaluate the achievement goals, objectives, and work plans.
  • Must have reliable transportation.
  • Must be legally authorized to work in the United States without sponsorship for current or future employment. GWH does not offer visa sponsorship for employment purposes.
Key Performance Indicators (KPI):

1. Serve GWH associates in assigned area(s).

2. Ensure 85% of the associates reach a 30-day retention.

3. Provide wrap-around service referrals to 90% of associates (cohort rollout determined by HR/DGR/WFD greatest need) to achieve a minimum of one (1) short-term goal identified in the needs assessment (IDP).

4. Complete and effective budget with the associates.

5. Provide all associates on the roster with referrals for co-enrollment into GWH Programs.

6. Execute WFD projects as assigned.

Physical Requirements/Work Environment:
  • Ability to sit, stand, bend, and reach.
  • Ability to lift, push, and pull up to 25 lbs.
  • Able to spend most working hours at a computer.
