Qualifications

• Degree in Nursing, Social Work, Public Health, or a related field required.
• Minimum 3 years of experience in oncology or chronic care coordination; survivorship experience strongly preferred.
• Strong project management, research, and organizational skills.
• Analytical skills to gather and summarize information for planning & development.
• Excellent communication and interpersonal skills, with the ability to work across departments and levels.
• Proficient in EMR systems and data reporting.

Responsibilities

• Lead the development, implementation, and ongoing management of a comprehensive cancer survivorship program.
• Serve as a key facilitator between clinical teams, patients, and community resources.
• Ensure survivorship care plans are meaningful, evidence-based, and scalable across the network.
• Coordinate program activities, education, and outreach within the breast clinic and beyond.
• Engage institutional leadership and contribute to research and resource development.
