POSITION SUMMARY:

We have an exciting opportunity to join our team as a Per Diem Registered Nurse - Short Stay/Telemetry, Days. The successful candidate practices in accordance with the American Nurses Association's (ANA) scope and standards of nursing practice. They utilize their clinical expertise to plan and deliver safe, therapeutic, patient-centered care through assessment, planning, implementation, and evaluation. The nurse works collaboratively within a multidisciplinary healthcare team to promote wellness and optimize patient outcomes, providing leadership by orienting, mentoring, and managing team members to ensure quality care with compassion, dignity, and respect. They participate in performance improvement initiatives impacting patient, family, and service outcomes.

JOB RESPONSIBILITIES:
  1. Performs other duties as assigned.
  2. Analyzes assessment data to determine diagnoses or issues.
  3. Identifies expected outcomes for individualized care plans.
  4. Develops strategies and alternatives to achieve expected outcomes.
  5. Implements the care plan.
  6. Coordinates care delivery.
  7. Employs strategies to promote health and safety.
  8. Evaluates progress towards outcomes.
  9. Practices ethically, guided by the Code of Ethics for Nurses.
  10. Maintains current knowledge and competence in nursing practice.
  11. Integrates evidence and research into practice.
  12. Contributes to quality nursing practice.
  13. Communicates effectively across formats and settings.
  14. Demonstrates leadership within the profession and practice setting.
  15. Collaborates with patients, families, and healthcare team members.
  16. Evaluates personal nursing practice against standards, statutes, rules, and regulations.
  17. Practices in an environmentally safe and healthy manner.
MINIMUM QUALIFICATIONS:

Must have a Bachelor's degree in Nursing (required for hires after 9/1/2020; grandfathered for those hired before). Current and valid New York State RN license and BLS certification. Knowledge of patient care standards. Excellent communication, organizational, customer service, and interpersonal skills. Basic computer skills. 1-2 years of nursing experience.

REQUIRED LICENSES:

Registered Nurse License - NYS

PREFERRED QUALIFICATIONS:

Bachelor's Degree in Nursing

Qualified candidates must communicate effectively at all organizational levels.

ADDITIONAL QUALIFICATIONS:
  • Bachelor’s degree and C.H.P.N. certification preferred.
  • Registered nurse licensed in the state.
  • Minimum of two years clinical management experience, including supervising nursing staff, preferred.
  • Minimum of three years hospice, home health, or related experience preferred.
