An established industry player is seeking an Assistant Patient Navigator to join their Enhanced Treatment and Recovery Program. Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. POSITION SUMMARY: We have an exciting opportunity to join our team as an Assistant Patient Navigator (Enhanced Treatment and Recovery Program). The Navigator will play a critical role in our Enhanced Treatment and Recovery (EnTRy) program, designed to improve early detection and treatment of mental illness, particularly psychotic-spectrum disorders, across the lifespan. The Navigator coordinates patient care services as well as provides outreach, education, and referral services to patients. The Navigator is responsible for carrying out tasks needed to execute the medical and support service plan. Depending on grant requirements and population served, the Navigator may provide rapid HIV testing and pre- and post-test counseling.
JOB RESPONSIBILITIES:
MINIMUM QUALIFICATIONS: To qualify you must have a College diploma required. Experience working with a behavioral health population required.
PREFERRED QUALIFICATIONS: Bilingual English/Spanish Bachelor's Degree.
