Job summary

Under minimal supervision, you will provide crisis intervention and counseling, advocate for patients, and collaborate with the care team to ensure seamless transitions and high-quality outcomes.

Qualifications

• Knowledge of case management components, crisis intervention and brief therapy processes.
• Ability to assess biopsychosocial needs and develop care plans.
• Experience coordinating care across inpatient and community settings.
• Strong communication and collaboration with multi-disciplinary teams.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and develop individualized care plans.
• Provide crisis intervention, counseling, and case coordination throughout hospitalization.
• Collaborate with internal/external resources to maximize patient outcomes.
• Lead and participate in family meetings and care conferences to align goals.
