Qualifications

  • 2+ years' experience in social services or related fields.
  • Valid driver's license and vehicle access.

Job description

The Support Coordinator is primarily responsible for coordinating support services to persons with intellectual disabilities and/or Autism and their families/caregivers. Major functions involve locating, coordinating and monitoring supports for individuals. There are 3 levels of Supports Coordinator: SC1, SC2 and Lead SC as well as Casual. Full time Supports Coordinator's will have the opportunity to advance to an SC2 or Lead SC based on education, experience, and performance.

Supports Coordinator Responsibilities:

  • All essential duties and responsibilities are completed within regulatory timeframes.
  • Complete timely and thorough documentation after each contact/service provided.
  • Meet productively goals and report accurately per departmental policy.
  • As requested, assist in emergency situations.
  • Attend supervision/meetings/training as requested/required.
  • Accurately record time and attendance via HRIS and Timekeeping system.
  • Document and report monthly expenses accurately and within agency policy expectations.
  • Model the organization's mission and values.
  • Other duties as assigned

Supports Coordinator Requirements:

  • Bachelor's degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science*; OR
  • 2 or more years' experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service*; OR
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service* and 1 or more years of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions**; OR
  • A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years' professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.
  • A valid driver's license along with access to a vehicle.
  • Proficiency with MS Office, computer/laptop, tablet and cell phone required

Working Conditions:

  • Flexible scheduling may be required.
  • Must be able to travel to the various office and community locations.
  • Work will be completed remotely at home, within the office, or other community location maintaining HIPAA compliance.
  • Ability to lift and carry light boxes of files and supplies (less than 10 pounds) on occasion.
  • A limited amount of walking and standing may be necessary in carrying out duties.
