Job summary

Commonwealth of Pennsylvania seeks a compassionate Supports Coordinator in Honesdale, Wayne County. You will help individuals with intellectual and developmental disabilities access services and stay connected to their communities, working with families and support teams to meet goals.

This is a full-time role with 8:00 AM to 4:30 PM hours, Monday through Friday, plus occasional overnight travel. Travel for field visits and training is occasional; telework is not available.

Qualifications

• Six months of experience as a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• A bachelor's degree with a social welfare major; or
• A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences and one year of professional social casework experience in a public or private social service agency; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences.

Responsibilities

• Service Planning: Develop Individual Support Plans and review them regularly to match each person’s needs
• Support Coordination: Connect individuals with natural supports and necessary services based on their goals
• Community Engagement: Encourage participation in community activities and employment when appropriate
• Field Visits: Travel to meet individuals and teams in person to review progress and needs
• Record Management: Complete documentation and data entry to maintain accurate and timely records
• Team Collaboration: Work closely with families, supervisors, and peers to support each individual’s growth

Education

Bachelor's degree with social welfare major

Bachelor's degree with 12 credits in sociology/social sciences
