Job summary

Benjamin Rose is seeking a dedicated Social Services Coordinator to connect residents with community resources and supportive services, promoting housing stability and well-being. Qualifications

  • Bachelor’s degree in social work or related field.

  • Experience working with vulnerable populations (e.g., homelessness, older adults, families, disabilities, survivors of domestic violence).
  • Strong assessment, case management, service planning, and crisis intervention skills.
  • Knowledge of aging-related issues and HUD requirements for senior housing programs.
  • Understanding of Fair Housing laws and professional boundaries.
  • Excellent interpersonal and collaboration skills.
  • Proficiency in Microsoft Office and video conferencing platforms.
  • Must have reliable transportation and ability to travel to meetings within the community.

Position Summary
  • Responsible for connecting residents/clients with community resources and supportive services to promote housing stability, self-sufficiency, and overall well‑being.
  • Work closely with property management, case managers, and external service providers to ensure residents have access to appropriate social, health, educational, and employment resources.
Essential Duties and Responsibilities

The following description is a general representation of the key duties and responsibilities of the position.

  • Conduct needs assessments for residents/clients to identify service gaps and barriers to stability.
  • Supports property management with review of applications, consulting on eligibility and participates in interview process of applicants.
  • Develop individualized service plans in collaboration with residents.
  • Provide referrals and coordinate access to community‑based services (e.g., healthcare, mental health services, employment programs, childcare, transportation, financial assistance).
  • Maintain strong partnerships with local service providers and community agencies.
  • Organize workshops, educational programs, and on‑site supportive activities.
  • Monitor resident progress and update service plans as needed.
  • Maintain accurate and confidential case notes and documentation.
  • Assist residents with crisis intervention and conflict resolution.
  • Track program outcomes and prepare required reports.
  • Support compliance with funder and regulatory requirements.
  • Advocate for residents to ensure equitable access to services.
  • Facilitate educational programs for residents and family members related to health, wellness, safety, financial assistance, resident rights or other appropriate information.
  • Report all suspected abuse situations to the Housing Manager and/or building property management.
  • Maintain current knowledge of Federal and State programs for older adults.
  • Establishes and maintains contacts and visibility in the community through professional affiliations, networking, meetings with advisory and/or advisory groups and others.
  • Represents management and the property in a positive and professional manner by maintaining positive relationships with residents, vendors and property staff.
Requirements
Specific knowledge, training or skills required to perform the duties of this position including specific courses, training programs or required certifications.
  • Bachelor’s degree in social work or related field.
  • Current and unrestricted Ohio licensure as an LSW.
  • Experience working with vulnerable populations (e.g., individuals experiencing homelessness, older adults, families, individuals with disabilities, or survivors of domestic violence).
  • Strong assessment, case management, service planning, and crisis intervention skills, including experience developing individualized service/action plans.
  • Knowledge of aging‑related issues including disability, mental health, abuse, and the aging services network, community resources, and social service systems.
  • Knowledge of eligibility and application processes for entitlement programs for older adults and familiarity with—or willingness to learn—HUD requirements for senior housing programs.
  • Understanding of Fair Housing laws, regulations, and legal liability considerations related to service coordination.
  • Excellent interpersonal, communication, and collaboration skills, with the ability to work both independently and as part of a team.
  • Strong organizational, time management, documentation, problem‑solving, and professionalism skills, with cultural humility and a commitment to combating ageism and promoting participant dignity and autonomy.
  • Proficiency in Microsoft Office, case management systems, and video conferencing platforms.
  • Ability to maintain confidentiality and appropriate professional boundaries.
  • Must have reliable transportation and the ability to travel to meetings within the community and to Benjamin Rose locations during work hours.
