Position Overview

The Supportive Living Companion works under immediate supervision, providing tailored services and activities within the consumer's home and community. The role involves ensuring services are delivered as designed to help consumers acquire, retain, and improve self-help, socialization, and adaptive skills, aligned with active treatment goals and objectives. Responsibilities include reporting incidents, completing activity logs linked to person-centered plans, submitting required documents, and maintaining compliance with personnel policies.

Work Schedule

SATURDAY & SUNDAY, 7:00AM-11:00PM

Essential Duties and Responsibilities
  • Deliver services and activities within the consumer's home and community based on their person-centered plan.
  • Support individuals in developing independent living skills, including home management, cooking, cleaning, socialization, and supported employment.
  • Follow incident procedures within 24 hours per Department of Human Services policy.
  • Complete activity logs as per the person-centered plan.
  • Report any changes affecting the individual receiving services.
  • Remind consumers to take or refill medication if necessary.
  • Complete and submit required documentation.
  • Meet initial and ongoing training requirements.
  • Maintain compliance with personnel documentation.
  • Perform other related duties as assigned.
Qualifications
  • High School diploma or GED.
  • One year of relevant experience in public health, human services, or community service, or two years working with individuals with developmental disabilities.
  • CNA or Medical Assistant certification qualifies for a special rate.
  • Ability to understand written activity plans, communicate effectively, access emergency and transportation services, pass background and maltreatment registry checks, and complete a drug screen.

Must pass background checks and drug screening. Employment offers are contingent upon these results.
