Job summary

Pathfinder, Inc. is seeking a dedicated staff member to provide instructional training to residents and help them acquire daily living and survival skills for independent living.

You will monitor training programs, collect data, and supervise residents in completing day-to-day tasks, ensuring a safe environment and smooth shift change communication.

Qualifications

• High School diploma or GED.
• 1 year of relevant supervised work experience with public health, human services or other community service agency; OR two years' verifiable experience with individuals with developmental disabilities.
• CNA Certification or Medical Assistant certification may qualify for a special entry rate.
• Must be able to pass applicable background checks and drug screen; offer based on results of checks.

Responsibilities

• Manages and supervises residents' active treatment program in accordance with individual program plans.
• Follows prescribed activities to meet individual objectives; provides services developed in the program plan.
• Initiates learning skills programs (reading, money management, cooking, grooming, dress, hygiene).
• Completes forms and reports including progress notes.
• Provides care and instruction for daily living skills (home management, cooking, cleaning, socialization) using behavioral techniques.
• Maintains a safe and healthy environment for residents.
• Offers community inclusion activity options for residents.
• Monitors and reminds residents to take and refill medications.
• Intervenes in residents' behaviors professionally and effectively.
• Encourages healthy work attitudes in residents who work.
• Attends meetings and training sessions as directed.
• Communicates information to incoming staff at shift change for continuity.
• Provides transportation to clients as needed for Pathfinder business.
• Performs other related duties as required.

Education

High School diploma or GED

POSITION 1: FRIDAY, 10:00PM-8:00AM; SATURDAY & SUNDAY, 8:00PM-8:00PM (OVERNIGHT)

POSITION 2: MONDAY-THURSDAY, 10:00PM-8:00AM (OVERNIGHT)

Essential Duties And Responsibilities

• Manages and supervises residents' active treatment program in accordance with their individual program plans, and demonstrates the skills and techniques necessary to implement individual program plans.
• Follows prescribed activities to meet individual objectives. Provides services which have been developed in an individual program plan.
• Initiates learning skills programs, i.e., reading, money management, cooking, grooming, dress, hygiene.
• Is responsible for the completion of appropriate forms and reports including recording progress notes.
• Provides care and instruction for all residents in the facility in all areas of daily living including home management skills, cooking, cleaning and socialization skills, utilizing behavioral management techniques.
• Assists in maintaining a safe and healthy environment for the residents.
• Provides a variety of choices in community inclusion activities for residents of the facility.
• Monitors/reminds residents when it is necessary to take and refill medications.
• Manages residents in appropriate behaviors in a professional manner and demonstrates the necessary skills and techniques to intervene effectively.
• Encourages healthy work attitudes and behaviors in residents who work.
• Attends meetings and training sessions as directed.
• Communicates verbal and written information regarding resident behavior, needs, medication, and other issues to incoming staff at shift change for program continuity.
• Provides transportation (in personal vehicle) to clients based on their needs and/or for Pathfinder business.
• Performs any other related duties as required or assigned.

Education And Experience

High School diploma or GED;
One (1) Year of relevant, supervised work experience with public health, human services or other community service agency; OR
Two (2) years' verifiable successful experience working with individuals with developmental disabilities.
CNA Certification or Medical Assistant certification will qualify for a special entry rate.

Salary: $12.66 - $13.16 per hour
