FLSA Status: Non-Exempt

Posting Salary Range:$33,820 - $43,497

Office Location: Remote in North Carolina- within 75 miles of Scotland County

What We’re Looking For:

The Support Transition and Reentry (STAR) Specialist provides direct support to recipients enrolled in the Trillium STAR care team. The STAR Specialist assists the STAR Coordinator with ensuring the member is supported and assists the coordinator in completing relevant tasks. The STAR Specialist assists to ensure the member is moved to ongoing Care Management services and supports. The STAR Specialist is responsible for working with the recipients care team to provide transition linkages including school, employment, housing coordination, community service opportunities and other identified treatment supports and services. This individual will work with community resources and services to meet the identified needs for recipients who are transitioning back to the community.

On a typical day, you might:

  • Meet with Justice Involved members in-person in Correctional Facilities and in-person in the community to facilitate care management activities.
  • Collaborate with STAR Coordinator on systems and processes
  • Assist with transition planning
  • Engage members in the process
  • Provide educational materials to recipient

Qualifications:

High School, GED

Experience:

One (1) year of experience in the Health and Human Services field, including experience working with any combination of the BH/MH/SU/IDD population.

OR

Be a person with (1) year lived experience with a behavioral health condition and/or substance use.

OR

One (1) year of experience in criminal justice or juvenile justice systems to include justice involved experience.

OR

Equivalent combination of education/experience.

Preferred Experience: (if applicable)

  • Preference given to applicants who have their own history of criminal justice system involvement, incarceration, or MH/SU recovery and education, training, and work experience that shows a history of learning and applying the skills needed to work with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds is preferred.

Must have a valid driver’s license

Must reside in North Carolina to be considered for remote status:Remote in North Carolina-Within 75 miles of Scotland County
