40 hours; Monday - Friday 9AM - 5:30PM

Location

This position will travel to see home/nursing home/assisted living patients in Buncombe, Haywood, Madison and surrounding counties.

Position Summary

The Support RN provides direct nursing care, and instructs the LPN and Hospice Aide staff in the delivery of nursing care to clients.

Support RN will perform 4-6 patient visits per day depending on location and acuity of patients served. This role may perform 2-4 admissions per days, depending on length of shift and complexity of patient. At times, they may receive assignments that are a combination of admissions & patient visits – adjusted accordingly depending on the mix of each.

Essential Duties & Responsibilities

  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
  • Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
  • Record and report the results of the nursing assessment.
  • Plan, initiate, deliver, and evaluate appropriate nursing acts.
  • Teach, assign, or supervise other personnel in implementing the treatment regimen.
  • Collaborate with other health care providers in determine the appropriate patient and family-centered plan.
  • Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
  • Provide teaching and counseling about the patient's health.
  • Report and record the plan for care, nursing care given, and the patient's response to that care.
  • Supervise, teach, and evaluate those who perform nursing functions and administer nursing programs and nursing services.
  • Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.

  • Must be willing and able to care for patients with infectious disease processes unless approved accommodations
  • Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
  • Knowledge of HIPAA privacy rules and regulations
  • Knowledge of electronic medical records systems

Minimum Qualifications

Experience:

  • Two years of clinical experience preferred

Licensures & Certifications: (if applicable)

  • Valid NC RN license
  • Valid Driver’s License and proof of automobile insurance

Summary Of Physical Requirements

  • Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
  • Flexion/Extension of arms/wrist
  • Hand dexterity to include gripping, pushing/pulling and writing
  • Lifting/carrying of 50 lbs. often

Other Duties

  • Participate in department and staff meeting as required
  • Participate in ongoing education and training offered by the agency and/or required by nursing supervisor as part of individual education plan of RN
  • Comply with company policies, procedures, guidelines, and Mission Statement
  • Practice safety and infection control methods
  • Assist in providing 24 hour on-call services, including coverage of at least one holiday per year
  • Assess for the need of other disciplines
  • Perform other but similar duties as assigned
