RN Case Manager
Reports To: Clinical Team Leader
Status: Non-Exempt Hourly
Employment Type: Full-time (FT, PT, PRN)
Shift: 40 hours; Monday - Friday 8AM - 4:30PM
Location
This position will travel to see home/nursing home/assisted living patients in Jackson, Macon, Graham, Swain, Cherokee, and Clay counties.
Position Summary
The Support RN provides direct nursing care and instructs LPN and Hospice Aide staff in delivering nursing care to clients. The role involves 4-6 patient visits per day, depending on location and patient acuity, and may include admissions and patient visits based on the schedule and patient needs.
Essential Duties & Responsibilities
- Practice within the scope of license, certification, or registration, as applicable, and as required by law and regulation.
- Assess patients' physical and mental health, including reactions to illnesses and treatments.
- Record and report nursing assessments' results.
- Plan, initiate, deliver, and evaluate nursing acts.
- Supervise and instruct other personnel in implementing treatment plans.
- Collaborate with healthcare providers to determine appropriate patient and family-centered plans.
- Implement prescribed treatment and pharmaceutical regimens.
- Provide health teaching and counseling.
- Document care plans, nursing care provided, and patient responses.
- Supervise and evaluate nursing staff and services.
- Ensure safe and effective nursing care delivery.
Knowledge, Skills & Abilities
- Willingness to care for patients with infectious diseases, with accommodations if needed.
- Knowledge of relevant regulations and Medicare Conditions of Participation.
- Understanding of HIPAA privacy rules.
- Proficiency with electronic medical records systems.
- Basic data entry, organization, interpersonal, and communication skills.
- Ability to work under pressure, take initiative, and collaborate effectively.
- Flexibility to work hours as needed and to participate as a team player.
- Skill in pain and symptom management, understanding reimbursement and regulatory requirements.
Minimum Qualifications
- Two years of clinical experience preferred.
Licensures & Certifications
- Valid NC RN license.
- Valid Driver’s License and proof of automobile insurance.
Physical Requirements
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling.
- Flexion/Extension of arms/wrist.
- Hand dexterity for gripping, pushing/pulling, writing.
- Lifting/carrying up to 50 lbs. often.
