Job summary

Proforma Health Inc seeks compassionate Direct Support Professionals (DSPs), caregivers, and CNAs to assist adults with intellectual and developmental disabilities in a community-based setting. Responsibilities include personal care, daily living support, medication administration, transportation coordination, and promoting dignity, independence and inclusion.

Follow person-centered plans, document services, and communicate with families and supervisors to ensure high-quality, compassionate care
