Our Supports Planners/Case Managers perform tasks to assess clients’ needs, strengths, and support networks to determine their goals. As a Supports Planner, you will serve as the primary contact with clients. You will receive remote training and access to tools and resources needed to perform your role, including information gathering, monitoring service plans, and telephonic care coordination. - Collaborate with social workers, nurses, and other professionals to assist clients with complex needs
- Educate clients about self-direction, access resources, and support transitions to independence

Qualifications
- Bachelor’s degree in health/human services or related fields (psychology, sociology, counseling, physical therapy, social work, etc.)
- Strong oral and written communication skills
- Proficiency in research and computer skills, including Microsoft Office
- Ability to write reports and interpret documents
- Effective communication with individuals of varied cognitive abilities
- Valid Maryland driver’s license and reliable transportation
- Ability to work independently, prioritize, and manage multiple tasks
