The anticipated salary range for this position is $68,501 - $100,123.

WHO WE ARE LOOKING FOR

Join our dedicated team at the Montgomery County's Area Agency on Aging - Support Planning Agency, providing Medicaid reimbursed services tailored to individual health and safety needs. Our program enables individuals to remain in non-institutional settings as long as possible. The Supports Planner assists with program enrollment, service plans, and monitoring services.

Responsibilities of the Program Specialist II include:
  • Providing case management and supports planning services for various programs, assisting with Medicaid and non-Medicaid home and community-based services, including eligibility and maintenance.
  • Developing and submitting annual and revised service plans based on assessments.
  • Contacting clients monthly and conducting quarterly visits to assess and monitor services.
  • Submitting reportable events and entering activities into the electronic case record.
  • Conducting options counseling for individuals transitioning from nursing homes to the community.
  • Assisting the supervisor as needed, including participating in calls and meetings.
  • Traveling throughout the county using a motor vehicle for assessments and case management.
  • Completing background checks and medical history reviews for selected candidates.
Minimum Qualifications
  • Experience: Two (2) years in Medicaid eligibility and funded programs.
  • Education: Bachelor's Degree in human services fields such as psychology, social work, sociology, nursing, or counseling.
  • Equivalency: A combination of education and experience may be substituted.
  • License: Valid driver's license from the applicant's state of residence.
Preferred Qualifications
  • 2 years of experience with Medicaid eligibility and funded programs.
  • 1 year as a Supports Planner using LTSS web-based systems.
  • 1 year of case management experience with individuals having complex medical or behavioral health needs.
  • Experience with Microsoft Office 365 products.
