Through our Support & Stabilization program, we provide intensive in-home supports to families involved with the Department of Children and Families (DCF), aiming to maintain or regain custody of their children. Our goal is to alleviate crises and help families stay intact, reducing foster care placements.

We seek a compassionate, resourceful Support and Stabilization Specialist to join our team. The Impact You'll Create

As a member of the child/family team, you'll collaborate with HopeWell Clinical and Supervisory staff, as well as DCF, using a child-centered, family-focused, trauma-informed approach. Your role involves providing therapeutic interventions, advocacy, guidance, and community connections. You will carry caseloads of children, youth at risk, and families involved with DCF, ensuring their safety and wellbeing in accordance with HopeWell and DCF policies.

Your Responsibilities
  • Provide in-home and community-based clinical support, skills, and guidance for children, adolescents, and families.
  • Conduct assessments of strengths, progress, and needs.
  • Develop strength-based treatment and safety plans with the treatment team.
  • Act as a secondary team lead in team-based service delivery to ensure a safe, affirming environment.
  • Maintain case files and documentation per regulations and policies.
  • Make referrals and connect families to community resources.
  • Assist caregivers in navigating systems and accessing supports.
  • Collaborate with DCF, community organizations, and service providers.
  • Attend meetings such as Foster Care Reviews, treatment planning, and discharge planning.
  • Provide transportation to visits, appointments, and court as needed.
  • Participate in after-hours on-call rotations, including emergency responses.
  • Ensure timely billing and documentation of expenses.
  • Engage in supervision, staff meetings, and trainings.
Qualifications
  • Bachelor’s degree in social work, counseling, psychology, marriage and family therapy, or related field.
  • At least 1 year of clinical experience working with children, adolescents, and families with behavioral, social, and emotional challenges.
Preferred Skills
  • Experience working with children, navigating community systems, and supporting families.
  • Knowledge of child development, mental health, family systems, and therapeutic interventions.
  • Ability to build trusting, empathetic relationships with families from diverse backgrounds.
  • Proficiency in basic computer skills and documentation.
  • Ability to work in home and community settings, including environments with pets.
  • Strong teamwork and collaboration skills.
Additional Requirements
  • Background checks as required by law.
  • Current trainings and licensure as needed.
  • Valid driver’s license and access to a vehicle.
  • Flexibility for evening, weekend, and emergency work.
  • Other duties as assigned.
