Qualifications

May act as Interim Rehab Coordinator with the following duties (Rehabilitation Services Only): Clinical rounding with Medical Director on patients referred to the Acute Rehab Unit and appropriate clinical decision making in consultation with Medical Director for admission decisions impacting the Acute Rehab Unit Completion of the Pre-Admission Screening on Acute Rehab Admissions Attend Interdisciplinary Conferences and facilitate the coordination of patient care Completes all IRF-PAI assessments including: Admission FIM and Quality Indicators Discharge FIM and Quality Indicators Reporting Therapy Minutes Assists in reviewing and revising department policies and procedures and/or developing new department policies and procedures. Manages and lead discipline-specific staff meetings. Collaborates with other rehabilitation department leadership to map out processes for performance workflow improvement. **Knowledge, Skills, and Abilities*** Must have good communication and organizational skills, proficiency in MS Office Products.
