Job summary

Golden Gate Regional Center is seeking a Supervisor Social Worker for the Self Determination Program in San Rafael. Hybrid schedule with local travel across San Francisco, San Mateo, and Marin counties and occasional statewide travel.

You will train and support staff and families, develop Individual Program Plans, monitor funding and progress, and ensure HIPAA compliance. A clinical license and CAL regional experience are preferred.

Qualifications

• Bachelor's Degree in Social Work or related human services field; Master's preferred.
• Valid Clinical License at time of hire and ability to maintain license in good standing preferred.
• 4+ years post-Master's work with increasing responsibility.
• 2+ years Regional Center Experience.

Responsibilities

• Be the main point of contact for SDP questions and training internally and externally.
• Initiate and respond to communications on service eligibility and standards.
• Collaborate with clients, families to develop and monitor program plans.
• Provide advocacy services and train individuals and families in advocacy.
• Maintain confidential client records and prepare reports.
• Participate in Person-Centered planning meetings and budgeting.
• Support SDLAC members and attend monthly meetings.
• Complete funding and related SDP budget reporting.
• May serve as Independent Facilitator coordinating SDP services.

Job description

Starting Salary Range: $75,518 - $90,621

GGRC is looking to hire a Supervisor Social Worker for our Self Determination Program to provide training, advocacy, service arrangements and carry out administrative functions in relation to the participants of the self-determination program. To monitor Self Determination Program (SDP) and report progress per the Self-Determination Program guidelines. To provide support and internal consultation on SDP to the ongoing social work unit on an as needed basis. To collaborate with and support GGRC's Self Determination Local Advisory Committee, as well as GGRC's fiscal and resource development units.

This position will be based out of our office in San Rafael in a hybrid schedule. Local travel within the Counties of San Francisco, San Mateo and Marin counties will be required. Very occasional statewide travel to sites throughout California. Access to reliable transportation permitting travel to and between many different locations throughout the day in order to perform the outlined responsibilities of the position.

Responsibilities

• Be the main point of contact person for questions and training related to the self-determination program both internally (with Social Work, Fiscal, Resource Development, Administration, etc.) and externally (with DDS, the Self Determination Local Advisory Committee (SDLAC), individuals and families)
• Initiates and responds to individuals and family supported, service provider, and outside agency communications on topics including service eligibility, service standards, methods, generic agency programs and problem solution
• Works collaboratively with clients, families and others to develop program plans, and monitors program results
• Provides advocacy services and trains individuals and families in advocacy issues
• Maintains required case records including funding and billing records and produces reports as required
• Engages in Person-Centered program planning meetings with individuals/families/independent facilitators/service providers to develop Individual Program Plans and individual budgets
• Acts as primary GGRC staff support and partner to the SDLAC and its members. Attends monthly SDLAC meeting.
• Adheres to monitoring requirements and completes progress reports, including to DDS
• Completes funding of related SDP budget with pertinent report completion
• May function as an Independent Facilitator (IF) and will be responsible for the coordination and monitoring of service delivery and progress per Self Determination Program guidelines
• Provides community information and advocacy related to Self-Determination Program, including providing, updating and monitoring content on GGRC's Self Determination Webpage. Responds to [e-mail] emails from community members and others.
• Perform other duties as assigned
• Serve as back up to Social Workers as needed
• Participate in the Supervisory On-Call rotation on a voluntary basis
• Participate in special projects, and assist with additional duties or tasks, as assigned
• Maintain all client information as confidential in compliance with the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the California Lanterman Act.

Requirements

Education/Experience
