Outposted Therapeutic Housing Units Program (OTxHU)

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.

Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). Located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high-quality clinical care for patients in custody with complicated health conditions. OTxHUs bridge the gap between jail care and inpatient hospitalization, with admissions and discharges based on clinical needs. CHS will be the primary healthcare provider, while the NYC Department of Correction will handle security and custody.

The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this groundbreaking project, with a planned opening as early as the end of 2024. This is an incredible opportunity to join a passionate team providing care to some of the City’s most marginalized and vulnerable populations.

*To support continuity of operations and care, staff may also be required to work in CHS locations within the jails. While CHS seeks highly qualified individuals, preference will be given to equally qualified internal candidates.

Position Overview

Job Description

The Supervisor of Nurses (SON) diagnoses and treats human responses to health problems through data collection, physical assessments, case-finding, health teaching, counseling, and providing supportive or restorative care. They execute medical regimens prescribed by licensed physicians, dentists, or other authorized healthcare providers, in accordance with regulations.

Under the guidance of the On-Site Director of Nursing (OS-DON), the SON directs and administers nursing care, coordinating activities with physicians, administration, pharmacy, clerical, and correctional staff.

The SON should have a thorough knowledge of nursing principles based on biological, physical, and psychosocial sciences, and possess leadership skills to guide nursing staff.

They assist the OS-DON in ensuring compliance with standards, policies, and procedures of NYC Health + Hospitals and the Department of Nursing. The role involves working with adult patients, including infants from birth to one year, and individuals aged sixteen to eighty.

Responsibilities

  1. Provide direct patient care as necessary, documenting progress using SOAP format.
  2. Coordinate nursing care by assigning staff, making rounds, assessing patient needs, and ensuring continuity across shifts.
  3. Evaluate and supervise nursing staff, including new hires, and conduct ongoing performance evaluations.
  4. Identify staff development needs and plan training strategies.
  5. Conduct staff meetings to promote communication and professional relationships.
  6. Monitor quality of care through assessment and improvement mechanisms.

EMERGENCY RESPONSE

  • Respond to medical emergencies, evaluate patient conditions, and provide appropriate care including CPR, AED, controlling bleeding, and preventing shock.
  • Coordinate patient transfers and document treatments.

COORDINATOR / SUPERVISOR RESPONSIBILITIES

  1. Familiarize with and implement nursing practice standards and policies.
  2. Delegate tasks to non-professional staff, providing guidance and supervision.
  3. Assist with staff precepting and ongoing education.
  4. Supervise medication administration and participate in quality reviews.
  5. Engage in professional development and research activities.

CARE OF THE ENVIRONMENT

  1. Maintain supplies and equipment, ensuring a safe environment.
  2. Observe security regulations and proper care of contraband.
  3. Promote a positive unit morale through effective communication and respect.

HUMAN RELATION SKILLS

  1. Assist colleagues, cooperate with staff and patients, and maintain professionalism.
  2. Accept constructive criticism and communicate effectively.

WORKING CONDITIONS

Clinic and infirmary environments with constant standing, walking, bending, lifting, and reaching. Ability to exert up to 100 pounds of force occasionally.

Qualifications

  1. Valid NY State license and registration as a Registered Professional Nurse; BLS certification through AHA.
  2. At least three years of nursing experience.
