The Supervisor of Care Management serves in a leadership role, supporting the Director with daily responsibilities. In collaboration with patients, families, physicians, and the interdisciplinary team, the Supervisor assists Case Managers and Social Workers in facilitating patient progress through the acute episode of care efficiently and cost-effectively. Responsibilities include monitoring the level of care, initiating evidence-based protocols and order sets based on admission diagnoses, influencing care progression, facilitating patient navigation, and proposing transition plans within 48 hours of admission. The Supervisor acts as an advocate for the Care Management Department and staff oversight. A Case Manager certification is required within one year of hire.
