Job Responsibilities

Promotes optimal clinical and resource utilization, facilitates appropriate lengths of stay and reimbursement, and acts as a key informational and educational resource for the team. Develops organizational approaches to problem solving, analyzes systems for improvement opportunities, and promotes quality of care through collaboration with team members, patients, and families.

Minimum Qualifications
Education, Certification, and Licensure
  • Master’s Degree in Social Work or related field.
  • LGSW/LCSW/LICSW certification in West Virginia.
Experience
  • Three (3) years of Medical Social Work experience.
OR
Education, Certification, and Licensure
  • Bachelor of Science Degree in Nursing.
  • Current unencumbered licensure with the WV Board of Registered Professional Nurses or appropriate state board, or multi-state licensure through eNLC.
Experience
  • Three (3) years of registered nursing experience.
Preferred Qualifications
  • Care Management Certification within one year of hire.
  • Two (2) years in inpatient hospital setting and care management or supervisory experience.
Core Duties and Responsibilities

Oversees discharge/transition planning, conducts weekly clinical-financial meetings, ensures compliance with regulatory and insurance requirements, participates in staff training, provides clinical guidance, documents interactions, supports competency development, and promotes a collaborative environment. Other duties may be assigned.

Physical and Work Environment

Prolonged walking, standing, or sitting may be required. Flexible hours including weekends and holidays. Reasonable accommodations are available for disabilities.
