Job summary

Integrated HomeCare Services (IHCS) is seeking a Clinical Care Coordination Supervisor to oversee the UM team and ensure compliant, cost‑effective care coordination for home health and DME services. You will lead referrals across health plans, drive staff development, and monitor performance to improve patient outcomes.

The role requires a BSN or related field, 3–5 years in UM/case management, and at least 2 years of leadership, with strong regulatory knowledge and Microsoft Office proficiency.

Qualifications

• Bachelor's degree in Nursing (BSN) or Healthcare Administration required or related field.
• 3–5 years of experience in utilization management, case management, or related healthcare setting.
• At least 2 years of leadership or supervisory experience in healthcare.
• Strong knowledge of CMS, NCQA, URAC regulations and compliance.
• Proficiency in Microsoft Office suite.

Responsibilities

• Oversee daily operations of the Utilization Management (UM) team, ensuring regulatory and organizational standards are met.
• Lead and support Referral Coordinators for multiple health plans; monitor performance and outcomes.
• Ensure compliance with UM processes; evaluate eligibility for benefit coverage; coordinate with internal/external stakeholders to optimize patient care.
• Collaborate with health plan case managers and claims payers to facilitate care coordination and accurate claims processing.

What you will be doing:

The Clinical Care Coordination Supervisor is responsible for overseeing daily operations of the UM team, ensuring compliance with regulatory standards, organizational policies, and industry best practices. This role supports staff development, monitors performance, and promotes high-quality, cost-effective care coordination. Leading and supporting a team of Referral Coordinators for various health plans. This role ensures compliance with utilization management (UM) processes, evaluates eligibility for benefit coverage, and collaborates with internal and external stakeholders to optimize patient care. Additionally, this position works closely with health plan case managers and claims payers to facilitate appropriate care coordination and ensure accurate claims processing.

Qualifications:

• Bachelor's degree in Nursing (BSN, Healthcare Administration, or a related field.
• Minimum of 3–5 years of experience in utilization management, case management, or a related healthcare setting.
• At least 2 years of leadership or supervisory experience in a healthcare environment.
• Strong understanding of utilization management, medical necessity criteria, and regulatory compliance (e.g., CMS, NCQA, URAC).
• Required to uphold compliance principles as outlined in the Code of Conduct and related policies. Actively supports and participates in mandatory Corporate Compliance Program training initiatives annually or as required.
• Proficient in Microsoft products (Word, Excel, Powerpoint)
• Excellent leadership, problem-solving, and decision-making abilities.
• Strong communication and interpersonal skills for cross-functional collaboration.

Associates or better in Health Administration or related field.
