Job summary

Kolmac Integrated Behavioral Health is seeking a PHP Therapist to deliver individual and group therapy within the Partial Hospitalization Program, coordinating with an interdisciplinary team. Qualifications

  • Master’s degree in social work, Counseling, Psychology, or a related field.
  • Minimum 2 years of experience in a behavioral health setting.
  • Experience in PHP or outpatient programs.
  • Knowledge of DSM-5 diagnoses and mental health disorders.
  • Licenses LMSW, LCSW, or LPC.

Responsibilities

  • Develop and update individualized treatment plans with the team.
  • Provide individual and group therapy at IOP and OP levels of care.
  • Deliver interventions using MI, CBT, trauma-informed care, and related modalities.

  • Maintain timely clinical documentation compliant with CARF standards.

Job description

The PHP Therapist at Kolmac Integrated Behavioral Health and all its subsidiaries ("Kolmac" or the " Company provides comprehensive mental health services to clients enrolled in the Partial Hospitalization Program (PHP). This role includes individual therapy, group therapy, assessments, and treatment planning in coordination with the interdisciplinary treatment team. The PHP Therapist ensures that all clinical documentation is completed accurately and on time, while maintaining a supportive, therapeutic environment that promotes patient recovery and well-being.

Responsibilities
  • Conduct comprehensive assessments, including biopsychosocial evaluations and diagnostic screenings using DSM-V and ASAM criteria.
  • Develop, implement, and regularly update individualized treatment plans in collaboration with the interdisciplinary team.
  • Provide direct care through evidence-based individual and group therapy sessions, including psychoeducational and addiction recovery groups at IOP and OP levels of care.
  • Deliver clinical interventions grounded in Motivational Interviewing, Cognitive Behavioral Therapy, trauma-informed care, and other best-practice modalities.
  • Facilitate case management sessions, including treatment goal setting, discharge planning, and collaboration with referral sources and external providers.
  • Educate clients on mental health conditions, treatment options, recovery strategies, and coping skills.
  • Coordinates care for allappropriate supportservices, including referral sources, family members, and other support providers.
  • Participate actively in treatment team meetings to review client progress and coordinate holistic care.
  • Provide crisis intervention andparticipatein a rotating on-call schedule to support patients in crisis after hours, as needed.
  • Maintainaccurate,timelyclinical documentation in alignment with organizational, state, and CARF standards.
  • Collaborate with psychiatrists, case managers, and other providers to meet the co-occurring psychiatric and substance use needs of patients.
  • Develop andmaintaina therapeutic alliance that is collaborative, individualized, and adaptable to evolving clinical needs.
  • Promote the organization’s image in a professional, supportive, and educational manner within the community and with referral partners.
  • Stay informed on current clinical practices, regulatory changes, and advancements in the behavioral health field.
  • Demonstrate a commitment to clinical excellence, as measured by outcomes such as patient retention, treatment goal achievement, successful program completion, and transitions between levels of care.
  • Obtain necessary secondary signatures on documentation from licensed supervisors when applicable.
  • Attend all required staff meetings,trainings, and professional development opportunities.
  • Perform other duties as assigned.
Qualifications
  • Master’s degree in social work, Counseling, Psychology, or a related field.
  • Minimum 2 years of experience in a behavioral health setting, preferably with PHP or outpatient programs.
  • Experience in conducting assessments, treatment planning, and therapeutic interventions.
  • Strong understanding of DSM-5 diagnoses and mental health disorders.
  • Knowledge of trauma-informed care, CBT, DBT, and other therapeutic modalities.
  • Excellent communication and interpersonal skills, with the ability to engage and support clients from diverse backgrounds.
  • Proficient in electronic health records (EHR) and clinical documentation.

Licenses and Other Required Credentials: LMSW, LCSW, LPC

The base pay for this position pays $34-44/hour. Factors, such as but not limited to, geographical location, relevant experience, licensure, education, and skill level may impact the pay for this position.
