Job summary

Qualifications

• Minimum 3 years’ experience in health services field required, SUD experience preferred.
• CCAR or MDHHS peer recovery coach certification required.
• CHW Certification, preferred.
• High School Diploma or GED required.

Responsibilities

• Advocate for clients and connect them with community resources.
• Identify and gather health, SUD, housing and financial resources.
• Provide problem-solving techniques with clients.
• Meet with and communicate with clients regularly.
• Support health promotion activities and education.
• Observe/participate in client assessments and action plans.
• Document activities and maintain records.
• Develop and maintain educational materials for staff and clients.
