Job summary

State of Utah in Provo, UT is seeking a Substance Use Disorder Counselor (SUDC) to join the Dual Diagnosis Treatment Team. You will coordinate services with community partners, provide recovery literacy through therapy, and document cases carefully to support treatment planning.

Qualifications

• Utah license with one of CSUDC/LSUDC/CASUDC/LASUDC/ACMHC/CMHC/LCSW.

Responsibilities

• Coordinate and refer services with community partners and families or patients.
• Provide information and recovery literacy through group and individual therapy.
• Prepare detailed case documentation including intake, history, risk assessments and service plans.
• Provide counseling to clients/residents which may involve family interactions.
