Job summary

Frontier Behavioral Health in Spokane is seeking qualified professionals for the Emergency/Transitional Team to support residents in emergency and transitional housing, focusing on housing stability and behavioral health, with collaboration across services and providers.

The role includes on-site support, intensive case management, and coordination of mental health, substance use, medical, and social services, travel to community sites, and documentation in accordance with agency policies.

Qualifications

• Ability to communicate and engage effectively with individuals from varied backgrounds.

Responsibilities

• Locate individuals on the HCA referral list.
• Provide SUD services and counseling sessions.
• Coordinate with Trueblood teams and be a clinical consultant on evidence-based practices.
• Assist clients with housing, jail, hospital referrals and community resources.
• Maintain confidential documentation and participate in multidisciplinary meetings.
