• Answers to Supplemental Questions (Who is your mentor teacher and what school site will you be student teaching?)
• Copy of Transcript (BACHELOR'S Degree transcripts ONLY - unofficial copies ARE acceptable)
• Credential Copy (Teaching Credential or 30-Day Substitute Teaching Permit (NO PROSPECTIVE PERMITS) SEE EXAMPLE UNDER LINKS RELATED TO JOB )
• Letter(s) of Reference (2 letters within the last 12 months)
• TB Screening Result (Within the last 4 years, if it is from a school district within the last 6 months)
