1. Provide a valid NJ Substitute School Nurse Certificate and complete online application.
2. Complete the Criminal History Review process with the Office of Student Protection. https://nj.gov/education/crimhist/
3. Provide Documentation of Mantoux test with date and results. Please provide your most recent Mantoux/TB result per guidance from the NJ DOE. "TB testing is not recommended for new employees, student teachers, and contractors of the district with a documented tuberculosis test result, regardless of when this test was done. Repeat TB testing is not required in the absence of known or suspected exposure to active and infectious TB disease or unless otherwise ordered by a licensed clinician."
