Job summary

North Country Healthcare in New Hampshire seeks a SUD Grant and Outreach Specialist to support NH Doorway SAMHSA-funded activities, ensure grant compliance, and improve access to outpatient SUD services in rural communities.

You will coordinate cross-departmental efforts, track performance metrics, and collaborate with regional Doorways, hospitals, and community partners to strengthen outreach and referral pathways.

Qualifications

• Bachelor's degree preferred or 5 years’ experience in public health, health administration, social services, communications, or a related field.
• BLS/ACLS or Heartsaver CPR AED certification renewal required for staff as applicable.
• Experience with grant-funded programs or SAMHSA grants preferred.
• Knowledge of screening, referral coordination, and care navigation in healthcare settings.

Responsibilities

• Support operational implementation of NH Doorway–aligned, SAMHSA funded activities, ensuring consistency with scopes of work, timelines, and performance measures.
• Assist with grant compliance including documentation, data collection, and progress reporting with the NCH Grant Coordinator.
• Track Doorway related deliverables such as screening and assessment volumes, referral coordination, appointment timeliness, and engagement outcomes.
• Serve as a project coordinator for Doorway initiatives, developing work plans, timelines, and task tracking tools.
• Facilitate cross-departmental collaboration with clinical, intake, quality, marketing, and administrative teams.
• Monitor progress toward grant milestones, identify barriers impacting access or flow, and elevate issues with proposed solutions.
• Coordinate outreach activities to strengthen Doorway access points, referral pathways, and community awareness.
• Build relationships with regional Doorways, hospitals, EDs, primary care, behavioral health providers, and state grant administrators.
• Participate in regional Doorway meetings, coalitions, and planning efforts.

Position Summary

Adhering to System Values, Service Excellence and Standards of Behavior, the SUD Grant and Outreach Specialist supports the implementation, coordination, and long-term sustainability of The Doorway Substance Use Disorder (SUD) treatment program at North Country Healthcare. This role ensures grant-funded activities are carried out in alignment with NH Doorway and SAMHSA requirements, while advancing timely access to outpatient SUD services for individuals in a rural community.

The Specialist works collaboratively across clinical operations, intake, marketing and communications, finance, community partners, and regional Doorway stakeholders to support grant compliance, project management, community outreach, and referral coordination. The position serves as a key liaison to strengthen access points, improve care flow, and enhance program visibility and effectiveness.

The anticipated hourly range for this position is $32.41/hr - $40.51/hr - $43.76/hr. Final compensation will be determined based on the selected candidate's applicable experience, skills, qualifications, and other job-related factors.

Education

ESSENTIAL QUALIFICATIONS

• Bachelor's degree preferred OR 5 years’ experience in public health, health administration, social services, communications, or a related field.

Certification

• All employees and contracted staff will have either successful completion of an American Heart Association* accredited course in Basic Life Support (BLS), or ACLS and PALS combined. (for clinical staff), Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis.

• Support development of impact stories, service summaries, and outreach content consistent with SAMHSA and NH Doorway communication standards.
• Assist in targeted outreach to referral sources and underserved populations in coordination with marketing strategies.
• Track data related to Doorway access, referrals, outreach activity, and service utilization.
• Assist NCH Grant Coordinator with preparation of SAMHSA progress reports, Doorway reporting, and internal leadership updates.
• Contribute to sustainability planning and continuous improvement for Doorway supported services.
• Performs additional duties as assigned.
