Job summary

North Country Healthcare is seeking a Substance Use Disorder Counselor/Clinician to deliver evidence-based assessment, counseling, crisis response, and recovery support within a New Hampshire Doorway program and integrated healthcare setting.

Responsibilities include screening, brief intervention, referral to treatment, and coordinating care with patients, families, and a broad network of providers to promote sustained recovery and access to services.

Qualifications

• Associates in Substance Abuse Counseling or equivalent. Masters preferred.
• Licensed Alcohol and Drug Counselor (LADC) must be able to practice in New Hampshire.
• BLS/ACLS/PALS or Heartsaver CPR training (policy dependent) and renewal on a regular basis.

Responsibilities

• Provide evidence-based assessment, counseling, crisis response, and care coordination for adolescents, adults, and families within a New Hampshire Doorway program and integrated healthcare setting.
• Screen, brief intervention, referral to treatment, and ongoing recovery planning.
• Document screenings, assessments, interventions, and outcomes in the electronic health record.
• Coordinate referrals and follow-up across continuum of care, including outpatient and intensive services.
• Collaborate with primary care and behavioral health teams to support integrated treatment planning.
