Job summary

Boston Medical Center is seeking a Discharge Planning Resource Specialist (Substance Use) to guide complex discharge planning in collaboration with Social Workers, Case Managers, and inpatient teams. You will manage a 20-35 patient caseload and coordinate post-detox placements with community facilities and methadone clinics.

The role requires a BSW or related degree, trauma-informed care experience, and strong communication skills.

Qualifications

• BSW or Bachelor’s in Human Services or related field required.
• MSW graduates encouraged to apply; relevant experience valued.
• Experience in human services, social work, or counseling, especially with substance use.
• Trauma-informed care experience and Medicaid application familiarity preferred.
• Strong interpersonal, written and verbal communication essential.

Responsibilities

• Manage discharge planning for patients with substance use needs.
• Collaborate with social workers, case managers and inpatient teams.
• Coordinate post-detox treatment placements and provider links.
• Oversee Medicaid applications and care coordination with families.
• Provide training and guidance to care managers and unit staff.
• Participate in quality improvement activities and policy adherence.
