Job summary

Haymarket Center in Chicago, IL is seeking a Counselor (CADC and/or clinical license required) to support adults in addiction and behavioral health treatment. The role includes facilitating groups, assessing patients, and coordinating care plans with timely documentation in charts.

The counselor will provide crisis intervention, refer for medical or psychiatric evaluation as needed, and collaborate with supervisors to ensure high-quality care aligned with Haymarket Center standards.

Qualifications

• High school diploma or GED required.
• Related substance abuse experience preferred.
• CADC licensure or licensure within 2 years of service.

Responsibilities

• Facilitates and monitors physical and emotional health status of adult patients in treatment.
• Provides appropriate referral when medical or psychiatric evaluation is needed.
• Facilitate didactic and therapeutic groups and document for each patient in attendance.
• Clinically assess and re-assess patients for level of care and document in charts.
• Provide treatment planning, referral, and aftercare planning with documentation.
• Maintain patient charts to meet QA standards and document progress clearly.
• Monitor medication self-administration and document medications dispensed.
• Monitor family visits/sessions and document in charts.
• Provide crisis intervention as required and follow up assessments.
• Attend patient staffing/interventions and document outcomes.
• Meet weekly with supervisor for clinical supervision; seek supervision when needed.
• Performs other duties as assigned by supervisor.
