Job summary

Harboroaks seeks a Substance Use Counselor to support treatment and recovery for patients with opioid use disorder. The role involves individual and group counseling, documentation, crisis intervention, and care coordination in a clinical setting.

Qualifications include a high school diploma or higher, 2+ years in counseling, and relevant degrees. Licensure or state certification may be required. Strong communication, empathy, and collaboration with care teams are essential.

Qualifications

• High School diploma or equivalent required; bachelor's in social/health services preferred; 2+ years in counseling required.
• OR bachelor's in other field required and four+ years in counseling.
• OR master's in social/health services required and 1+ year professional experience.

Responsibilities

• Conduct individual and group counseling sessions; document as required.
• Co-facilitate group/family sessions and assist with behavior modification.
• Perform initial and follow-up assessments for patients.
• Prepare individual treatment plans for each patient.
• Ensure timely and clear documentation of patient care, treatment and incidents.
• Provide crisis intervention and plan for aftercare as needed.
• Act as liaison between referral sources and patients; assist with insurance communications.
• Possibly perform case management duties to ensure quality care.
