Job summary

Responsibilities

• Work with LCAS to help deliver SAIOP services, including UDS, facilitating group and coverage for LCAS.
• Co-facilitate SAIOP groups weekly with the program counselor.
• Support patients in accessing care resources including referrals to behavioral health programming, dental care, detox, recovery services, and crisis services when appropriate.
• Remind patients of scheduled group therapy and other appointments; track SAIOP hours and refer to step down services.
• Adhere to AMH policies and procedures; maintain respectful environment and ethical communication with staff and patients.
• Attend mandatory AMH meetings and Behavioral Health Team meetings.
• Coordinate services with the Community Resource Advocate to improve stability; refer to CRA for needed resources.
• Collaborate with AMH teammates to provide wrap around services to enhance patient care.
• Maintain the CADC/CADC-I license.
• Ensure timely, accurate documentation in the patient’s medical record for each encounter.

JOB SUMMARY

The primary responsibility of the Substance Use Care Coordinator is to assist the Substance Abuse Intensive Outpatient Program (SAIOP) Counselor with the coordination of services across all levels of patient care. The Substance Use Care Coordinator will support the needs of patients in addiction recovery through providing care coordination through the different levels of care, and assisting in organizing and hosting daily groups, as well as support the clinical lead in care delivery during groups. Individual applicants with experience working within the mental health and substance use treatment field are preferred.

KEY ACCOUNTABILITY #1

• Work with LCAS to help deliver SAIOP services. This includes providing UDS to group members, facilitating group when needed and providing coverage for LCAS.
• Co-facilitate SAIOP groups weekly with the program counselor.
• Support patients in accessing care resources including referrals to behavioral health programming, dental care, and referring to supportive programming like Detox, Recovery Services, and Crisis Services when appropriate.
• Support patients by reminding them of scheduled group therapy and other appointments, tracking SAIOP hours and support referral to step down services.
• Adhere to AMH ’s policies and procedures; and supports an environment based in mutual respect, using ethical communication with staff and patients.
• Adhere to AMH Employee Code of Ethics and maintain professional boundaries with staff and patients and will consult with site supervisors with any questions or concerns.
• Will attend all mandatory AMH meetings, and Behavioral Health Team meetings.

• Work with the Community Resource Advocate to coordinate services to assist patients in increasing stability factors in their lives through accessing community needs by referring patient to a CRA to establish needed resources.
• Works in collaboration with AMH teammates to provide excellent wrap around services to enhance patient care.

KEY ACCOUNTABILITY #3

• Maintain the CADC/CADC-I license.
• Maintain timely, accurate, and patient specific documentation within the patient’s medical record for each unique encounter with a patient.

• Adhere to AMH ’s policies and procedures; and supports an environment based in mutual respect, using ethical communication with staff and patients.
• Adhere to AMH Employee Code of Ethics and maintain professional boundaries with staff and patients and will consult with site supervisors with any questions or concerns.

Requirements

EDUCATION/EXPERIENCE

• Bachelors or equivalent
• 2+ years’ experience in the Mental Health and Substance Use Treatment field.

PERFORMANCE REQUIREMENTS

Knowledge:

• Experience and knowledge working with individuals with substance use disorders, and mental health conditions.
• Familiarity with group therapy and running therapy groups.
• Experience and knowledge of community resources, and public benefits.

Skills:

• Excellent interpersonal communication and community building skills.
• Skilled at time management, well organized, with ability to meet deadlines, and respond to staff and patient requests in a timely manner.
• Proficient with technology, with experience in documenting in Electronic Health Records.

Abilities:

• Ability to read, understand and follow program guidelines and service materials.
• Willingness to work within the clinic schedule and be available for the clinic during hours of operation.
• Openness to accepting feedback and willingness to adjust where it is necessary for program adherence and staff and patient safety.
• Strict adherence to all Federal and State regulations as it applies to healthcare, health information, patient care, and documentation of patient care.

PHYSICAL REQUIREMENTS

• Seeing: Must be able to read written and printed material, as well as use computer, fax machine, Xerox machine and other office equipment.
• Hearing: Must be able to hear well enough to communicate with coworkers and others. Good hearing is necessary to receive detailed information through oral communication and to make fine discriminations in sound.
• Sitting: Must be able to sit for the majority of the work shift while answering telephone and communicating with others.
• Standing/Walking/Mobility: Must be able to stand intermittently and be capable of walking on hardwood and linoleum floors with concrete underpayments. Must be able to walk between various clinical areas and departments.
• Fingering/Grasping/Feeling: This position requires manual and finger dexterity and eye-hand coordination for easy and skillful use of hands when working with and performing normal office duties and responsibilities.
