Job summary

Centurion Health's MHM Services (FACT program, California) seeks a Substance Abuse Counselor to provide intake, assessment, and counseling for individuals with substance use disorders in a community setting. You will develop treatment plans, conduct group and individual sessions, coordinate aftercare, and maintain accurate clinical records in compliance with DSH requirements.

CADC certification preferred; CPR certification required; valid driver's license; background check and clearance may be

• CADC certification preferred, but will accept other Alcohol and Drug Counseling Certifications.
• CPR certification is required.

Responsibilities

• Provide intake, assessment, treatment planning, group and individual counseling to clients in the Substance Use Disorders/Addiction program.
• Provide after-care plan for clients returning to the community including program referrals to substance use disorders/addiction treatment.
• Maintain accurate and thorough clinical records per Company and DSH requirements in a timely manner.
• Provide input and reports to the Clinical Supervisor regarding the clinical needs of clients and programming, improving ways of offering prevention, outreach, and treatment services.
• Work collaboratively with behavioral/mental health and medical staff when clients have co-morbid conditions.
• Communicate with treatment and case management staff through daily briefing and debriefing between shifts.
• Attend required staff meetings, safety training and in-service programs as required.
• Identify and maintain professional boundaries to preserve the integrity of treatment and the professional relationship.
• Proactive in performance improvement activities as demonstrated by participating in the Continuous Quality

MHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals-Forensic Assertive Community Treatment Program (FACT). The FACT program is a statewide system that provides an unlocked, supervised living environment with 24-hour staffing. FACT's model of care is centralized services with a slow transition to community-based services. Services needed to support community reintegration include but are not limited to the following:

• Forensic mental health treatment through individual and group therapy settings
• Life skills training
• Day socialization groups
• Collateral contacts
• Home visits
• Substance abuse screenings
• Psychiatric services
• Case Management
• Court reports
• Psychological assessments

Under the direction of the Program Director the Substance Abuse Counselor is responsible for providing support, guidance, and mentoring the patients in a community setting to meet service delivery requirements of the FACT Program.

QualificationsQualifications

• Certified Alcohol and Drug Counselor (CADC) certification preferred, but will accept other Alcohol and Drug Counseling Certifications
• Must be able to obtain CPR certification
• Must be able to pass background investigation and obtain agency security clearance where applicable
• Valid State Driver’s License
