Job summary

Pinnacle Treatment Centers, Inc. is seeking a COE Case Manager to coordinate care, manage cases, and connect patients with community services.

The role involves ongoing follow-up to maintain a high-quality patient care experience. Responsibilities include assessing referrals, admitting patients to MMT as appropriate, coordinating with counselors and external providers, and contributing to discharge planning and after-care referrals.

Qualifications

• Associate, bachelor's or master's degree in Human Services or related field.
• Ability to travel up to 25% locally as needed.

Responsibilities

• Coordinate services with counselors, medical teammates, and external providers.
• Monitor cases, verify attendance, and advocate for needed services.
• Communicate patient progress in interdisciplinary meetings.
• Assist with discharge plans and after-care referrals.
• Travel as necessary to assist with referrals and community education.
• Attend team meetings and complete required training.

As a COE Case Manager, you will provide coordination and connection to other community services and agencies. This role provides ongoing case management and follow-up for patients to maintain a positive patient care experience.

Requirements:

• An associate, bachelor's or master's degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services)
• Ability to travel up to 25% locally as needed

Responsibilities:

• Assess new referrals for appropriate level of care and SDOH/HRSN.
• Admit new patients to MMT as appropriate.
• Determines need for therapeutic medical, psycho-social, and psychiatric evaluations.
• Coordinates services with counselors, medical teammates, and external providers.
• Monitors cases by verifying patient attendance, observing, and evaluating response to treatment, advocating for needed services, obtaining additional resources, intervening in crises and providing universal support.
• Communicating patient progress through interdisciplinary meetings as scheduled.
• Assist with discharge plans and after-care referrals.
• Assist other Pinnacle COE programs via telehealth or in person as needed.
• Provide resources to other team members.
• Participate in required annual training.
• Travel as necessary to assist with referrals, to assist and support patients in the community, and educate the community on services provided.
• Follow a teamwork philosophy when needed to help cover absent teammate duties.
• Attend team meetings and complete all training courses timely as required.
• Other duties as assigned.

Education

Required

Bachelors or better in Human Services or related field.

Preferred

Associates or better in Human Services.

Experience

Preferred

• Previous experience in case management and/or utilization review preferred.
