Job summary
Astrana Health, Inc. is seeking a Registered Nurse for the Sub-Acute Care team at Foothill Regional Medical Center in Tustin, CA.
This full-time role offers an hourly compensation of $41.00 to $62.00 and requires a CA RN license, BLS, PALS, ACLS, and 1+ year in sub-acute/related settings. You will assess patients, administer medications, coordinate discharge planning, and collaborate with interdisciplinary teams to deliver patient-centered care.
Qualifications
- CA RN license required
- 1+ year experience in Sub-Acute/Skilled Nursing/Rehab/Long-Term/Telemetry/Acute Care
- BSN preferred
- Strong communication and critical thinking skills
- Ability to work varied shifts including days/evenings/nights/weekends/holidays
Responsibilities
- Assess, monitor, and evaluate patient conditions and initiate interventions
- Administer medications, treatments, and procedures per physician orders
- Utilize clinical resources to provide safe, high-quality care
- Monitor outcomes and adjust care plans for optimal recovery
- Coordinate discharge planning and referrals as appropriate
- Collaborate with physicians, therapists, case managers, and support staff
- Participate in interdisciplinary team meetings and care conferences
- Delegate and supervise patient care activities as appropriate
- Serve as a clinical resource and role model for staff
