Job summary

Qualifications

• Bachelor’s degree in social work, counseling, or related field required.
• Master’s degree preferred.
• 3–5 years relevant experience in education or social services preferred.
• Citizenship, residency or work visa required.

Responsibilities

• Coordinate the Act 264 process and facilitate interagency meetings.
• Provide home visits to help families access services and improve attendance.
• Assist with developing attendance plans.
• Serve as liaison between home and school in welfare or foster cases.
• Develop awareness of school policies and connect parents with resources.
• Respond to questions from parents/guardians and resolve conflicts.
• Provide mental health crisis support referrals.
• Coordinate Wrap Around Services for intensive social/emotional needs.
• Maintain confidential student data and records.
