Vista Life Innovations seeks a Senior Student and Family Advisor for the Discover Program in Westbrook, CT. The role focuses on counseling, case management, and preparing students for independent living in a supervised dorm setting.

The position emphasizes lead case management, family collaboration, and coordinating medical, wellness, and financial service needs to build essential life skills.

Qualifications

• Master’s degree in Special Education, Social Work or related field is required.
• Bachelor’s degree plus 5 years of relevant experience may substitute for a Master’s degree.

Responsibilities

• Develop individualized IPPs and IPs for each student/member.
• Coordinate goals and service needs across four areas: social, vocational, life skills, community involvement.
• Serve as liaison between students, families, and the Discover Program.
• Counsel families during transition to independent living with clear communication.
