Qualifications

• Associate degree in nursing is required.
• Minimum of three years hospital or acute-care nursing experience.
• Current RN license and BLS certification required.

Responsibilities

• Lead coordination of Stroke and Sepsis programs and ensure compliance with standards.
• Attend activations to observe clinical response and educate care team.
• Coordinate data abstraction, validation, and submission for measures.
• Prepare reports and dashboards for leadership and committees.
• Collaborate with physicians, nursing, EMS, and departments to coordinate care.
• Develop and monitor corrective actions and performance improvement plans.
