Job summary

Hackensack Meridian Health seeks an experienced APN/PA to join the Stroke Team, providing patient-centered care and leading interprofessional collaboration. You will assess patients, order/interpret tests, manage pharmacologic therapies, and educate patients and families.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate in Nursing (APN).
• Maintains clinical competencies through CE and practice.
• NPI required and prescriptive authority per NJ law.
• Collaborative agreement with physicians.
• Excellent written and verbal communication.
• Proficient with MS Office/Google Suite.

Responsibilities

• Perform comprehensive health assessments and document history/physicals.
• Order, perform, and interpret labs and diagnostics with physicians.
• Manage general medical/surgical conditions with pharmacologic knowledge.
• Prescribe medications safely under NJ regulations and provide education.
• Promote APN practice recognition through academic and clinical activities.
• Deliver patient/family-centered care and advocate for patients.
• Adhere to NJ laws, HMH policies, and standards of care.
• Participate in interdisciplinary rounds and care coordination.
• Engage in clinical research and quality improvement projects.
• Mentor APNs and precept students in APN programs.
