Job summary

Hackensack Meridian Health is seeking an Stroke Team Advanced Practice Nurse (APN) or Physician Assistant to provide patient-centered, coordinated care in collaboration with the practice/hospital team. The role emphasizes leadership, direct clinical practice, and quality improvement activities in a high-performing stroke program.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• National Provider Identifier (NPI).
• Institutional and regulatory documentation is in place.

Responsibilities

• Performs and documents accurate history and physical assessments appropriate for age and clinical problem.
• Orders, performs, and interprets laboratory tests and diagnostic studies in collaboration with physicians.
• Manages general medical and surgical conditions based on clinical knowledge.
• Prescribes medications safely in accordance with guidelines and hospital policy.
• Promotes APN practice recognition through conferences, publications, and presentations.
• Provides patient and family-centered education and care.
